Page 295, left-hand side column, 4 lines up from bottom. "9.25"
should read "9.24"
Page 276, right-hand side column: The equation "O = R*Y" should
read as "O = ii*Y". And in the statement immediately below the equation
defining terms, the associated definition should now read as: "ii* is
an n x n matrix with elements of i along the diagonal and zeros elsewhere".
Pages 284-285. The statement of the solution algorithm is incomplete. A
fuller version is given in the attached document.
