"Notation", page xvii: One commonly used greek letter, lambda, is missing from the list. This symbol is used mainly to denote a Lagrange multiplier (or sometimes a vector of lagrange multipliers), and so is interpretable as a shadow price (or prices) in an optimisation problem.
"Introduction", page 13: At p13, near the bottom
left column, the text says "This chapter includes....matrix
algebra...' where the chapter being referred to is chapter 9. This is wrong.
The matrix algebra
appendix is now an appendix to Chapter 6.
